School Year Information & Forms

2024-2025 Annual Records Information

Please be sure to complete the Annual Student Update Online form on the Skyward Family Access Portal to update student Health Information, Contact Information, and to sign off on school policies including:

• Handbooks 
• Student/Teacher/Parent/Guardian/Administrator Compact (print, complete, and sign) 

Other forms that need to be completed by families if applicable:

• Free & Reduced Meals — Apply Online
• Volunteer Interest Form

Optional: Student Accident Insurance

Please complete and return the Student Insurance Enrollment Application if you would like to enroll your student in an optional accident insurance plan.

• Insurance Information & Enrollment Application (Please contact the main office if you would like to apply for this benefit.)

Skyward Family Access Information Portal

Please be sure to check and confirm that all your information is correct and up-to-date in Skyward.

• Confirm all emails, phone numbers, addresses, emergency contact persons and other important student information.
• Visit the Parent Portal/Skyward Information Page for Skyward information.

Change of Address (if applicable)

If you have moved or are moving, please be sure to fill out a Change of Address packet or fill out the online form. Transportation cannot be changed until required paperwork is submitted. Please note that changes in transportation can take 3 to 5 business days and transportation in two different districts may not be granted by the district for two separate households. If you choose to download the packet, please submit your completed packet to Kristy Grace, Kristy.Grace@rak12.org.

Transportation cannot be changed until required paperwork is submitted: valid proof of residency (utility bill: energy/water, affidavit, lease or deed), Charter School Enrollment Form, Change of Address Form, and updated Emergency Contact. Please note that changes in transportation can take 3 to 5 business days and transportation in two different districts may not be granted by the district for two separate households. If you are moving to a new school district, you must register with the district also.
